Many colleges ask for a medical fitness certificate at document verification or on reporting day. It is one page, but a missing seal or registration number can send you back to a clinic on hostel move-in day.
What should a medical fitness certificate for college admission include?
There is no single national format; if your college gives you a form, use that one. A useful model is the medical certificate for NEET-UG 2025 qualified candidates published by IPGMER Kolkata for MBBS and BDS admissions in West Bengal. It asks the doctor to record:
your identity, verified from Aadhaar, voter card, passport or a school or college ID
height, weight, BMI, pulse, blood pressure and chest measurements
visual acuity in each eye, whether you use glasses or lenses, and colour blindness
immunisation status against the National Immunization Schedule
heart, lung and abdominal findings, and any neurological, orthopaedic, surgical or psychiatric illness
a final statement: fit to join the course, or needs further assessment
The form ends with the doctor's signature, registration number, council of registration, contact number and seal, plus your passport photo attested by the doctor.
Who can sign a medical fitness certificate?
For most courses, a Registered Medical Practitioner: a doctor registered with a state medical council or the National Medical Commission. Indian Maritime University's admissions FAQ is typical: candidates joining its non-marine courses submit a General Physical Fitness Certificate issued by a Registered Medical Practitioner.
You can check a doctor's registration on the NMC Indian Medical Register by name, registration number, year of registration or state medical council. Some colleges may ask for a certificate from a government doctor instead, so read your admission letter first.
What extra rules apply to MBBS and BDS?
Most candidates need only the form above. PwBD (persons with benchmark disabilities) candidates for MBBS go through a separate process. The NMC guidelines on assessment of PwBD for MBBS admission, dated 27 July 2026, base eligibility on an assessment of the candidate's functional competencies, not solely on the disability percentage shown on the UDID card or other disability certificate. The same guidelines rename the certificate issued by the designated boards the "Eligibility Certificate" and record that DGHS expanded the designated assessment centres from 16 to 61. This certificate comes from those boards, not from your family doctor.
What medical certificate does the merchant navy need?
The Indian Maritime University FAQ says marine course candidates need a Physical Fitness Certificate from a doctor approved by DG Shipping. Approved doctors are listed in the DG Shipping register of approved medical examiners of seafarers, kept under the Merchant Shipping (Medical Examination) Rules, 2000.
Tolani Maritime Institute's medical requirements for B.Tech Marine Engineering list unaided distance vision of 6/12 in each eye, or 6/9 in the better eye and 6/18 in the other, and state that colour blindness is not permitted, tested with the Ishihara chart.
How does the NDA medical examination work?
No private certificate replaces the military medical. The UPSC notice for the NDA and NA Examination (II), 2026 says candidates recommended by the SSB are examined by a Board of Service Medical Officers, and only those declared fit are admitted; unfit candidates can request an Appeal Medical Board, then a Review Medical Board. The standards are in the Joint Order Manual of Medical Standards for Entry into Armed Forces.
The notice advises fixing minor problems early and lists common ones: ear wax, deviated nasal septum, hydrocele or phimosis, being overweight or underweight, an under-sized chest, piles, gynaecomastia, tonsillitis and varicocele. Permanent tattoos are allowed only on the inner forearm or the back of the hand, with case-by-case exceptions for tribal custom. What should you check before reporting day?
Download the exact form from your college or counselling portal.
Book the exam as soon as your seat is allotted, leaving time for a repeat test or specialist opinion.
Carry photo ID, passport photos, glasses or lenses, old prescriptions and your vaccination record.
Before leaving the clinic, check that your name and date of birth match your marksheet, every field is filled, and the signature, registration number, council, seal, date and attested photo are all there.
Look up the registration number on the NMC register; for merchant navy, confirm the doctor is on the current DG Shipping list.
Check whether your college wants a recently dated certificate, and keep copies, a scan and the original.
At the clinic you can say: "I need a medical fitness certificate on this college form. It needs your registration number, council and seal, and my photo attested. Could you also note vision for each eye and colour vision?"
